Dear Fellow Skiers, 

I kindly inform and remind you that on Sunday, February 18, 2024, XXXIII Polish Physician Alpine Ski Championships will take place in Wilmot Mt., Wisconsin under the patronage of the Consulate General of the Republic of Poland in Chicago. 

• Registration inside the main building: 12:15 pm-12:45 pm 
• GS (giant slalom) course inspection: 12:45 pm-1:15 pm 
• Start of the first run: 1:15 pm-1:30 pm 
• Start of the second and third runs immediately after the first run
• Conclusion and award ceremony: start around 3:00 pm-3:30 pm 

As in previous years, it is necessary to sign a “Waiver of Liability” stating that you participate at your own risk. 

If it is not too much trouble, please print, fill out, and bring this form to the event. 

This will expedite and streamline the registration process.
