I kindly inform and remind you that on Sunday, February 19, 2023, the XXXII Polish Alpine Skiing Championships for Doctors will take place in Wilmot Mt., Wisconsin under the patronage of the Consulate General of the Republic of Poland in Chicago.

Registration on site: 12:15 pm – 12:45 pm

Course inspection: 12:45 pm – 1:15 pm

Start of the first run: 1:15 pm

Start of the second and third runs immediately after the first run

Conclusion: 3:00 pm

As in previous years, it is necessary to sign a waiver of liability stating that you participate at your own risk. If it is not too much trouble, please print, fill out, and bring this form to the event. This will expedite and streamline the registration process.
